The disadvantage of these procedures is that they require the dentist to grind your tooth down to a tiny peg, which is very aggressive and traumatic to your tooth. It not only grinds off all the sick areas of the tooth, but it also grinds off all of the healthy area of the tooth. Current technology is awesome! You can now fix your tooth with very conservative, longer lasting tooth colored “Inlays and onlays.” These restorations are terrific in that they do not grind your tooth down to a peg, may actually strengthen your tooth (by the simple fact of removing less tooth structure), and fix only what is broken, leaving your healthy tooth structure intact. And as an added feature, they are virtually “Invisible” to the naked eye, unlike the porcelain crowns found currently in traditional dentistry. They are metal free. Rubber dam.

For many years, the standard for detecting decay in a tooth was the dental xray, or using the “dental explorer” whereby the dentists “picks” at your tooth to find soft spots (decay). The drawbacks of these methods are that traditional dental xrays emit more 50% or more radiation than the digital xrays we utilize. Instead of the “sharp explorer” to “pick” at your teeth, we prefer to utilize a dental laser, which detects decay in advance, sometimes before digital xrays. Poking a tooth with a sharp instrument is essentially not the standard to detect decay, as you can “contaminate” other teeth with the decay from the tooth previously checked, or you can press too hard and penetrate the enamel, essentially opening up a hole for food and sugar to further promote decay.. With digital xrays and the dental laser, we can conserve vital and important tooth structure, reducing the need for more invasive procedures such as crowns and root canals.

However, if the cavity is large, this may not be possible. But for the most part, it is a phenomenal advance in dentistry, and is a terrific technology to offer our guests.

Gum disease can wreak havoc on the mouth and the entire body, but thanks to a simple DNA test dentists can accurately diagnose the illness and provide proper treatment.

FLOWER MOUND, TX — Scientists and doctors are just starting to understand the true nature of gum disease. Gum disease, also known as periodontal disease, is an infection to the gums that initially has no major symptoms. But, while individuals are ignoring minor symptoms, gum disease is silently wreaking havoc on the mouth and even other areas of the body.

In advanced stages, gum disease can cause bad breath, bleeding gums, inflammation, loose teeth, and a receding gum line. That’s just the mouth. Research has shown that gum disease can double your risk for heart attack, triple your risk for stroke, worsen diabetes, and also contribute to systemic diseases such as Alzheimer’s disease, cancer, and other seriousness illnesses.

Thankfully, gum disease is a highly treatable illness. There are several tests dentists use to screen for gum disease. There is the basic oral exam, where dentists look for bleeding gums, inflammations, and infections. There is also a probe test, where dentists push on teeth to determine the stability of the gums. However, only one test accurately diagnoses the type of bacteria that has caused infection. This test is a Bacterial DNA test.

According to AirPerio, a research company dedicated to studying periodontal disease, there are 11 periodontal pathogens most commonly linked to gum disease, and the DNA test can readily verify which strain has infected the mouth. Other examinations and tests can verify the presence of gum disease, but there are many strains of bacteria that cause varying levels of gum disease.

“In cases where periodontal disease is likely present, it is imperative to perform a DNA test on the patient,” said Flower Mound dentist Dr. Jonathan J. Golab. “DNA testing verifies the type of bacteria involved, which allows dentists to accurately prescribe the correct antibiotic. This is crucial in cases that require antibiotic treatment, as it in avoids complications that come with incorrect medicine.”

Why is DNA Testing Important?
When determining the presence of gum disease underneath the gum-line, a DNA test is the only sure-fire diagnosis method commented Dr. Golab. In advanced or severe stages of gum disease, the bacteria will have moved underneath the gum-line to tooth roots and even bone. In these cases, antibiotics are required, but administering the proper antibiotic depends on which strand of bacteria the patient is suffering from.

Furthermore, according to AirPerio, if a dentist treats gum disease with the wrong antibiotic, the bacterium can become stronger, antibiotic-resistant strains, which makes removal of the bacteria even harder.

Procedure
The Bacterial DNA test is a quick, simple procedure. During the test, paper points are placed under the gum line for approximately 15 seconds. During this time, the paper absorbs bacteria under the gum-line.

Each strain of bacteria has unique identifying qualities that lab technicians can readily identify. After studying the test samples, results are sent back to the dentist, who can administer the proper treatment, be it antibiotics or other treatment such as laser therapy, scaling and root planning (SRP), or surgery if necessary commented Dr. Golab, a Flower Mound dentist who uses dental lasers.

Who Should Be Tested
There are several conditions that make individuals susceptible to gum disease. Genetics plays a big part, as well as dental hygiene, hormonal changes, medical history, and immune system strength all contribute to gum disease vulnerability.

“The truth is that 85% of adults have some form of gum disease,” said the Flower Mound Dentist. “But out of all those individuals, a majority won’t come in for an exam for any number of reasons. Gum disease is preventable and highly treatable, but so often patients wait too long to come in for a screening.”

Though gum disease is hard to recognize in its early phases, there are several symptoms. According to the Consumer Guide To Dentistry, these symptoms include: bleeding gums, sensitive or swollen gums, bad breath, or loose teeth.

Treatment
Depending on the severity of the condition, there are several methods to treat gum disease. Generally, there is SRP, laser therapy, and antibiotics.

According to http://www.simplestepsdental.com/, SRP is the process in which tartar and plaque are removed from tooth surfaces and roots. Plaque is the breeding ground for bacteria associated with gum disease, therefore removing plaque from the mouth is the first step in preventing or treating gum disease commented Dr. Golab. Also, since plaque fastens easier to rough surfaces, root planing smoothes irregularities of the root surface.

Laser therapy and antibiotics are adjuncts to SRP. Laser therapy utilizes various wavelengths that target bacteria and remove remaining bacteria cells after SRP. Antibiotics also attack bacteria, but attack white blood cells at the same time. This leads to a weak immune system and opens the body to other illnesses during treatment.

“Unless the patient needs surgery, treating gum disease provides patients with healthier smiles, stable gums, and an overall healthier body,” said, Flower Mound dentist Dr. Golab, who does Dental Implants. “The best defense is good hygiene and prevention. Often, patients feel awkward about making an appointment to check out swollen gums or plaque, but it’s far better to be safe than sorry.”

In addition to periodontal exams and treatment, Dr. Golab is a certified Flower Mound teeth cleaning and general dentist, providing services such as cleanings, tooth colored fillings, porcelain veneers, crowns, dental implants, and bridges. Dr. Golab also provides cosmetic dental work such as tooth whitening, gum contouring, laser enhancement, and smile enhancements.

Texas dentist Dr. Jonathan Golab is keeping his dental practice small to better serve his patients and maintain a warm atmosphere.

FLOWER MOUND, TX — Flower Mound dentist Dr. Jonathan Golab maintains a small, private, and more conservative practice atmosphere to provide a more comfortable facility for patients.

In comparison to larger practices, Dr. Golab has opted to keep his business small and patient oriented over larger patient rosters and shorter, impersonal appointments.

Dr. Golab and his team emphasize accessibility. Whether it’s a question about hygiene or dental treatment, the team is always available to help, especially in a time of need, commented Dr. Golab.
“Patient comfort is a variable that every practice should consider,” said Flower Mound dental care expert Dr. Golab. “Some practices use warm blankets. At my practice we focus on creating a bond with our patients. It may be old fashioned, but our patients are comfortable, and that is a great asset to the practice.”

In addition to general dentistry, Dr. Golab is a provider in Flower Mound who does cosmetic dental procedures such as veneers, dental crowns, bridges, smile designs, full mouth rehabilitation, implants, bonding, inlays and onlays, gum contouring, periodontal hygiene treatment, metal-free dentistry.

“Time is a big part of our practice,” said Dr. Golab. “We take the time to talk patients through their fears before treatment and answer questions. We take time to get to know our patients beyond their teeth and gums. We take the time to make our patients feel right at home in the dental chair. Maybe we could squeeze a few extra appointments in the day, but we’d rather give our patients a little elbow room.”

Dr. Golab recently created a list of the top five reasons patients should consider dental implants over other alternatives.

Tuesday, May 19, 2009- FLOWER MOUND, TEXAS – Flower Mound general dentist who performs cosmetic dental procedures Dr. Jonathan Golab is a proponent of dental implants to address a wide variety of dental maladies.

Dr. Golab recently created a list of the top five reasons patients should consider dental implants over other alternatives.

Do No Harm Philosophy
The philosophy of “do no harm” is a mantra in the world of holistic dentistry, Golab said.
Today’s forward-thinking dentists focus on being conservative and preserving as much of the natural teeth as possible when performing dental repairs.

This is possible with dental implants because unlike other solutions to missing teeth, only the area of the missing tooth is affected by treatment.

The Standard of Care
Dental implants have become the “standard of care” when replacing missing teeth, Golab said. Other repairs such as dental bridges require neighboring healthy tooth structure to be grinded down to provide an anchor for the bridge.

Bone Preservation
Dental implants actually preserve the jaw bone in the area of the missing tooth or teeth. When a tooth is lost or extracted, the bone in that area will begin to slowly wear away over time if left untreated, Golab said.

Leave the area untreated for too long and it becomes necessary to perform a bone augmentation procedure prior to placing a dental implant. Bone augmentation requires the use of synthetic bone or healthy bone that has been harvested from another area of the body and grafted to the jaw.

“It takes a while for grafted bone to integrate itself into the jaw area,” he said. “That means of restoring the appearance of the teeth will take longer when this is the treatment method.”

Preservation of Natural Teeth
Dental implants do not harm adjacent natural teeth. The same cannot be said for dental bridges. When a dental bridge is used to replace a missing tooth, the dentist must prepare the teeth on either side of the missing tooth by grinding them down to allow room for crowns to be placed on them, Golab said. These crowns serve as the anchor for the artificial tooth that will rest where there the missing tooth once was.

A Lifelong Repair
“Dental implants should certainly outlast you, whereas dental bridges will certainly need replacement at some point,” Golab said.

Tuesday, March 31, 2009- FLOWER MOUND, TEXAS – Registered Dental Hygienist Janette Jurek, who works for Dr. Jonathan Golab, DDS, recently was accepted to dental school at the University of San Antonio.

Jurek has worked for Golab, a Flower Mound general dentist who performs cosmetic dental procedures since August 2008. She starts dental school in July.

“I’m very proud of Janette,” Golab said. “This has been a dream of hers for some time, and I know she’s going to be a phenomenal dentist.”

Jurek initially considered dental school in her first year of work after having obtained a bachelor’s degree in dental hygiene in 2002. She spent the past two years working on pre-requisite courses and applied to three Texas dental schools. She was accepted to two, and chose University of San Antonio because “they were the first and most eager to accept me,” she said.

Once dental school starts, Jurek will have to quit her job at Golab’s office.

“It’s a very rigorous eight-to-five program five days a week,” she said. “There’s a lot of studying on top of it because you take well into twenty to thirty credit hours per semester.”

The program is four years and if Jurek opts to specialize in periodontics, which she is considering, that could require an additional two to four years of education.

“Being a hygienist, my heart is in periodontics,” Jurek said.

Golab “wasn’t surprised at all” by Jurek’s acceptance into the program, Jurek said, adding, “He knew in my resume that was my intention.”

Jurek asked Golab to be her mentor.

“He’s been a great teacher,” she said. “It’s hard when you have your own patients, but he’s still tried to make time for me. We shoot e-mails back and forth and text each other on the weekends.”

Jurek had never worked for a cosmetic dentist prior to being hired by Golab, so she didn’t know a lot about the specialty.

“I definitely didn’t know how the health of the gums played such a large part in the strength and long-term prognosis of restorative work,” she said. “That has validated my job and the patients’ job as far as home care. He really opened my eyes up about this.”

Jurek said she also has admired Golab’s ethics and “how particular he is” about his work.

“It’s really nice to see someone care so much about their patients and their work to do it right the first time,” she said.

You can’t have a healthy mouth without healthy gums. Sadly, 75% of Americans have some form of periodontal (gum) disease.

If your gums bleed when you floss (and you do floss - don’t you??), you have gum disease. Let me ask you this: what other part of your body that bled when touched, would you NOT be concerned about. Well, you need to be concerned about gum disease, because it is an INFECTION that causes the destruction of the bone that supports your teeth.
Fortunately, our bodies are able to take care of infections relatively easily, if given the chance. Think of gum disease, like you would of a deep cut with dirt and bacteria lodged within it. The cut would get badly infected! But, what if you cleaned the cut out and disinfected the wound? Assuming that you are otherwise healthy, the area would quickly heal. The gums are no different. It is important to clean up the gums, BUT it is also of major importance to support the bodies immune and regenerative systems.
Our job is to remove the deposits and toxins that have accumulated on the root surfaces (root planning & scaling) and to kill the bacteria in the pockets (herbal anti-microbial irrigation). We then educate you about proper oral hygiene procedures, as well as prescribe for you a specific regimen of hygiene and nutritional protocols.
We can only create the conditions for your mouth to return to health, it is up to YOU to do the rest. Remember; gum disease cannot be cured, it can only be controlled.
Sometimes people laugh, when I tell them that bacteria do not cause disease. Think about it, when do most people get sick? It’s not when the temperature outside is 95 and all those little bugs have ideal conditions for frolicking about. No, it’s when the weather gets nasty, during the holidays, or when you are traveling. In other words, it’s when your IMMUNE SYSTEM is compromised. It’s not the bugs - its how your body is able to handle the bugs.
If we were to ignore the condition of your body, we would simply be treating the SYMPTOMS, instead of the CAUSE.
Unfortunately, treating symptoms is very profitable for today’s medical & dental profession, not to mention the pharmaceuticals companies. Our goal is to get you healthy, and to TEACH you to stay that way. That’s what being a DOCTOR is all about.

It has been said that the eyes are the windows to the soul. But is your mouth the window to the rest of your body?

Increasingly, researchers are discovering that the condition of your teeth and gums can be an indicator of your overall health.
For example, patients who have severe gum disease, or periodontitis, are much more likely to have a buildup of plaque in their arteries. But it would be a leap of logic to say that if you don’t brush your teeth, you’ll have a heart attack. Scientists still are trying to explain the connection between the two diseases. It is unknown whether or not it’s cause-and-effect, or if they stem from the same pathophysiological process. In other words, there could be a common factor that’s causing both problems.
But cardiologists long have known that gum disease can cause bacteria to get into the bloodstream, and sometimes those bacteria can get lodged in a defective heart valve. This is why people with heart defects usually have been advised to take preventive antibiotics before having a dental procedure.
There is a growing body of research on the role of inflammation in heart disease. It turns out that a major cause of inflammation is chronic infection. It is certainly possible that ongoing gum disease could be putting people at risk for heart attacks.
This emerging research is beginning to break down the artificial barrier between doctors and dentists. The old view, held by both patients and health professionals, was that the teeth were separate from the rest of the body. Treatments performed on teeth were considered cosmetic and not related to the patient’s medical condition.
Insurance companies almost see teeth as a luxury. But the teeth are connected to the body by blood vessels and nerves, just like any other part of the anatomy. I wish the insurance industry would acknowledge this reality. Sometimes we see patients who don’t heal as well after a procedure, and we begin to think they may be diabetic. Studies have revealed a strong correlation between gum disease and diabetes. Often the severity of your periodontal disease is related to the severity of your diabetes. If you can control one, you can control the other.
Diabetic patients are susceptible to infections, so it may not be surprising that they would have a higher risk for gum disease. But new research has shown unexpected connections between oral health and other systemic conditions.
For instance, pregnant women who have gum disease are more likely to have low-birth-weight babies and to go into labor prematurely. Poor oral health doesn’t necessarily cause these pregnancy complications. It could be that both conditions are related to the patient just not taking good care of herself.
Regardless of the cause, an astute doctor should take a look at their patient’s mouth. If the condition of the teeth indicates that the patient is probably not brushing, it’s likely that he or she has other health issues, such as smoking, poor diet or lack of exercise. A more holistic approach is gradually evolving, both in dentistry and medicine. I do think people are recognizing the connection.
